When Goldfinch Health needed off their previous patient-messaging vendor, we advised against another point solution and built the chat where the clinical data already lived. We architected a real-time, HIPAA-conscious patient–nurse chat on Platform Events and CometD inside Experience Cloud — read receipts, attachments, scheduled messages — and migrated years of message history in ourselves. Then we added Gemini-powered live translation so nurses type English and Spanish-speaking patients read Spanish, plus a record-access audit trail. Five years on, it's still the system their care team runs on.

Real-time EHR sync on 38-field Platform Events — zero swivel-chair entry

A multi-state psychiatric health practice was running intake on duplicate data entry between Salesforce and their EHR. We advised an event-driven architecture and built it: EHR webhooks land on Apex REST endpoints that publish 38-field patient Platform Events; Flows subscribe and update Person Accounts and appointments in real time, while outbound triggers push new patients back to the EHR with MRN writeback and full error logging. We wired their voice-AI phone agent into the same pattern — AI-answered calls become structured Leads — and rolled out Shield Platform Encryption over PHI fields.

Closed Won to EHR patient record — 12 API callouts, one automated pipeline

For a national addiction-treatment network, admissions used to end at Closed Won and restart as re-keying in the clinical EHR. We built the bridge: one Queueable orchestrating 12 idempotent API callouts — patient, episode of care, documents — with token caching and environment switching in custom metadata. For a multi-facility behavioral-health treatment center, we rebuilt the number the whole business runs on: a daily census that recalculates itself when admit or discharge dates move, delivered with architecture briefs so their team owns it.

A 291-field clinical pre-assessment, rendered to archivable PDFs — no managed package

A virtual mental-health provider needed intake documentation that clinicians could trust and auditors could retrieve — without a document-generation license. We modeled the pre-assessment as a 291-field data model with eight related clinical objects, then built a record-type-aware rendering engine: flow-invoked Apex assembles the packet, Visualforce templates render each of four intake types, and the finished PDF files itself against the patient journey. Native platform economics, clinical-grade output, and a data model their next feature builds on instead of around.

Is your Salesforce work HIPAA compliant?
Compliance is a property of your program, not a line in a consultant's proposal — so we don't claim it, we build for it. Do we need Health Cloud?
Not necessarily. Every platform on this page runs on a custom patient data model on core Salesforce, and several replaced processes Health Cloud wouldn't have modeled well. Health Cloud earns its licensing when its prebuilt clinical model matches how you operate. How do you integrate our EHR with Salesforce?
Event-driven, not point-to-point. Inbound, your EHR's webhooks hit an Apex REST endpoint that does exactly one job: publish a Platform Event. Subscriber Flows do the writes asynchronously, so bursts never hit governor limits. Outbound, triggers and invocable Apex push new patients back with MRN writeback. Field mappings live in custom metadata, every callout is idempotent, and every failure writes an error log — because silent failure is worse than failure.
How do we know who accessed a patient record?
You log it. For a surgical-care client we built a record-access audit trail on Platform Events — an invisible component publishes open and close events, a trigger persists them for reporting — capturing 34,860 access events to date. It won't replace a full compliance program, and we'll tell you exactly what it does and doesn't prove, but it answers the question every healthcare org eventually gets asked.
